Physical well being: 8
Spiritual well being: 6
Psychological well being: 7
There are changes I need to make that will make me physically healthier, one of which is to completely quit smoking. I have been able to cut down, but have not quit yet.
Spiritually I try every day to get closer to God. This will be a life long journey and I believe that the longer I stay on this road the more inner peace I will have.
Psychologically, I am pretty well grounded, but I need to work on my ability for focus on the positive.
2. Develop a goal for yourself in each area (physical, spiritual, psychological).
Physical: Quit smoking
Spiritual: Allow time for me
Psychological: Practice seeing the good
3. What activities or exercise can you implement in your life to assist in moving toward each goal?
This just came to me. I can use all three areas to achieve the goal of quitting smoking. I have to decide that this is what I am going to do (psychological) and I have to have faith that I can do it (spiritual). Once I have done those two I will have achieved the goal of no smoking (physical).

I had to complete this exercise twice. The first time I got so relaxed I fell asleep. The second time my body became so relaxed that my cramps went away. My mind is usually darting in every direction, but with guided meditation I am able to quiet it and focus. It was very beneficial.
